Anyone have any link or what to write down onto a form for the church address, hotel address, flower girls name, best man name, etc ?
i would like to design one for myself and be more professional…hope theres something..

just group everything into general headings. I use something like the below designed in a PDF doc :
Names
Contact Numbers
Date
No. Of Guests
Locations (preparations / ceremony / reception/meal)
Travel Distance (distances between preparations / ceremony / reception/meal)
Times (preparations / ceremony / reception/meal)
Bridal Party (include page boys and flower girls here)
Brides Family
Grooms Party
Grooms Family
Group Shots
NOTES
You can edit until you get something that works for you
